Style(9) cleanup.

- Convert K&R-style function definitions to ANSI style.
- Remove `register' keywords.
- Use stdarg.h instead of varargs.h for variable numbers of arguments.
- #define _KERNEL_STRUCTURES when accessing kernel structures.
- No functional changes.
