libiconv was declaring a base kobj_class structure instead of an extended
kobj_class structure (which it calls iconv_converter_class).  Add a new
KOBJ macro to allow an extended structure to be declared and explicitly
require the full name of the extended structure to be specified by the
caller.

This bug was causing sysctl -a to crash the system when libiconv.ko is
loaded.

I'm growing tired of having to add #include lines for header files that
the include file(s) I really want depend on.

Go through nearly all major system include files and add appropriately
#ifndef'd #include lines to include all dependant header files.  Kernel
source files now only need to #include the header files they directly
depend on.

So, for example, if I wanted to add a SYSCTL to a kernel source file,
I would only have to #include <sys/sysctl.h> to bring in the support for
it, rather then four or five header files in addition to <sys/sysctl.h>.

Improve seperation between kernel code and userland code by requiring
that source files that #include kernel headers also #define _KERNEL or
_KERNEL_STRUCTURES as appropriate.

KObj extension stage IIIa/III

Merge inheritance support from FreeBSD:
  * Add multiple inheritance to kobj. Each class can have zero or more base
    classes and if a method is not found in a given class, its base classes
    are searched (in the order they were declared). This search is recursive,
    i.e. a method may be define in a base class of a base class.

KObj extension stage I/III

Isolate the reference counting for kobj classes in special functions to
allow clean locking in the next step.

Merge all calls of kobj_class_compile either into the new
kobj_class_instantiate or into kobj_init and make it static. Same for
kobj_class_free.

Remove kobj_class_compile_static, it wasn't used and is pretty pointless
since the kobj framework is not used before the VM subsystem has been
initialized.

Merge FreeBSD rev. 1.8:
Simplify the KOBJOPLOOKUP macro for the non-debug case so that gcc's
heuristics do not overestimate the code size quite so much.

anyway.
