For a datapoint: NT's HAL implements in*() and out*() (well, READ_PORT_UXXXX and WRITE_PORT_UXXXX) as out-of-line functions on x86; the rationale, I believe, is that I/O operations take far longer than the call itself.

On Alpha, though, they were in-line, presumably because it didn't have a
separate I/O space.
